Can I choose my provider?

What should I bring to my first visit?
Bring your photo ID, insurance card, medication list, and any relevant medical records. Review our patient forms and call the office if you need help preparing.
Is a Medicare wellness visit the same as an annual physical?
No. A Medicare Annual Wellness Visit focuses on a personalized prevention plan and is different from a routine physical exam. Ask the office which appointment fits your needs and confirm coverage with your plan.
